GIP and GLP-1: the incretin hormones behind Mounjaro
GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) are two naturally occurring gut hormones known as incretins. They’re released from cells lining the intestine within minutes of eating, and their job is to help the body manage the incoming nutrients β signalling to the pancreas, brain, stomach and fat tissue that food has arrived.
GLP-1 is produced by L-cells, found mainly in the lower small intestine and colon. When food arrives, GLP-1 stimulates the pancreas to release insulin β but only when blood sugar is elevated, which is why it carries a low risk of hypoglycaemia on its own. It also suppresses glucagon (the hormone that tells the liver to release stored glucose), slows the rate at which the stomach empties so sugar enters the bloodstream more gradually, and acts on appetite centres in the brain to increase feelings of fullness and reduce hunger. It’s this combination of effects β steadier blood sugar plus reduced appetite and food intake β that made GLP-1 the target of earlier medicines such as semaglutide (Wegovy/Ozempic).
GIP is produced by K-cells in the upper small intestine and was actually the first incretin discovered. Like GLP-1, it boosts insulin release in a glucose-dependent way, but it also acts directly on fat tissue, improving how the body stores and processes fat, and appears to enhance insulin sensitivity. Emerging evidence suggests GIP also works within the brain to reduce appetite and, notably, to reduce nausea β which may partly explain why combining it with GLP-1 can improve tolerability.
In people with type 2 diabetes and obesity, the natural incretin response is blunted, and the hormones themselves are broken down within minutes by an enzyme called DPP-4. Tirzepatide (Mounjaro) was designed to solve both problems: it’s a single engineered molecule that activates both the GIP and GLP-1 receptors and resists breakdown, so one weekly injection provides continuous incretin signalling. Targeting both receptors at once appears to produce a synergistic effect β clinical trials have shown greater reductions in blood sugar and body weight with tirzepatide than with GLP-1-only medicines, which is why it’s often described as a “dual agonist” or “twincretin”.
